I'm 6' 5" 245lbs and I usually get XL T, but I wanted something a little more form fitting so I went with the Large Tall. I was surprised that this was a little baggy after washing. I don't think a Med would fit, so I guess if you have a spare tire to hide I'd order your normal size. Still a quality shirt.
